Clinically reviewed blog agent for healthcare software companies on Salesforce, with PHI-safe sourcing and tiered claim review
The Content Operator turns de-identified customer questions, sales objections, published research and search and AI-answer gaps into briefed, drafted, SEO and AEO-structured blog posts, routing every clinical or outcome claim to a named reviewer before anything publishes. It lets a healthcare software marketing team move from two posts a month to one per business day without hiring, producing more pages that rank and get cited in AI answers, tracked to influenced pipeline in Salesforce.

What The Content Operator does
A mid-size healthcare software company selling patient engagement and care coordination tools to health systems and specialty clinics publishes twice a month. The bottleneck is not writing but sourcing and review. Good topics sit in support tickets and call notes that may contain patient details, and every draft that mentions outcomes, readmissions or clinical workflows waits weeks for the one clinician on staff. Leadership wants daily publishing without adding headcount or compliance risk. The Content Operator owns the content pipeline end to end for the marketing team: mining de-identified source material for topics, writing briefs in the brand voice, drafting posts built for both search rankings and AI citations, keeping a claims ledger that links every factual and clinical statement to a source, and running editorial QA. It sorts each draft into a review tier so operational posts move fast and clinical posts get proper sign-off. It never publishes a clinical claim on its own judgment, and it stops work when it detects possible PHI.
Custom build
The company runs Salesforce, not HubSpot, so HubSpot Breeze content agents are not a natural fit, and the hard parts here are custom: PHI redaction before any model call, a tiered review workflow and a claims ledger tied to an evidence library. A custom Claude build handles long-form drafting in a consistent voice and strict source-grounded QA well, and can be run through a model provider or cloud platform that signs a BAA. n8n orchestrates the pipeline and can be self-hosted so source material stays inside the company's environment, Airtable holds the evidence library and editorial board that reviewers actually use, and Salesforce stays the system of record for pipeline influence.
What starts it
- The content lead approves the weekly topic slate in the editorial board, which queues one brief per business day
- A monitored source publishes something relevant (a new clinical guideline, a payer or CMS rule change, a peer-reviewed study in the company's focus area), which creates a candidate topic and a refresh check on existing posts
- Recurring questions surface in de-identified sales call summaries or Salesforce opportunity notes, such as repeated objections from health system IT or compliance buyers
- A clinical or compliance reviewer returns a draft with edits, which restarts the agent on revisions
- A published post loses rankings or stops appearing in tracked AI answers, which triggers a refresh brief
What it reads
- Salesforce (opportunity stages, closed-lost reasons, campaign influence and content-touched opportunities)
- De-identified call and support summaries, passed through a PHI redaction step in n8n before the agent ever sees them
- Approved evidence library in Airtable: published studies, guidelines, product documentation and pre-approved claim language with reviewer and expiry date
- Keyword and ranking data from Moz plus Search Console for the company site
- AI answer visibility from Rank Prompt and OmniSEO showing which buyer questions cite competitors instead of the company
- Brand voice guide, style rules and existing published posts for internal linking
How it works, step by step
- Sanitize and gather source materialn8n pulls call summaries, support themes and Salesforce notes, runs them through a de-identification step that strips names, dates of service, record numbers, facility-patient combinations and other identifiers, and quarantines anything flagged as possible PHI instead of forwarding it. Only clean, aggregated themes reach the model.
- Pick topics and write the briefThe agent matches buyer questions against Moz keyword gaps and Rank Prompt and OmniSEO answer gaps, then writes a brief: target query, the questions an AI answer would need resolved, audience (clinical leader, operations director, IT or compliance), angle, internal links, and the evidence items from the Airtable library it plans to use.
- Assign a review tierEach brief is tagged Tier 1 (operational or how-to, no clinical or outcome claims), Tier 2 (references published research or regulations), or Tier 3 (patient outcomes, clinical efficacy, product performance or anything near treatment advice). The tier decides who signs off and is visible on the editorial board.
- Draft in AEO-ready structureClaude drafts in the brand voice with a direct answer in the opening paragraph, question-based subheads, short definitional passages AI engines can quote, a FAQ block, schema markup, and named author and reviewer bylines. Every factual sentence is tied to a row in the claims ledger with its source.
- Run editorial QAA second Claude pass checks the draft against the ledger (no unsupported claims, no outdated studies, no figures that differ from the source), the style guide, reading level, banned phrases (such as cure or guarantee language), link health and on-page SEO. Failures go back for revision automatically up to two times, then to a human.
- Route for sign-offTier 1 goes to the content lead for a quick edit. Tier 2 adds a compliance check on regulatory references. Tier 3 goes to the clinical reviewer with the claims ledger side by side so they review claims, not prose. Reviewer decisions and edits are logged.
- Publish, track and refreshApproved posts publish to the WordPress site on WP Engine with schema intact, a Salesforce campaign is tagged for influence tracking, and the agent watches rankings and AI citations. Declining posts and posts whose cited evidence has expired get queued for refresh.
When a person takes over
- Possible PHI detected in source material: the item is quarantined and the privacy officer reviews it, because the agent cannot judge whether a detail is identifying and should not try
- Any Tier 3 claim about outcomes, clinical efficacy or product performance: the clinical reviewer approves, edits or cuts it, since only a qualified person can stand behind clinical statements
- A post that references an FDA, CMS or payer rule, or could be read as promoting a regulated product use: compliance or regulatory affairs signs off before publish
- A draft that fails QA twice, or a topic the agent marks as sensitive (pricing, competitor comparisons, customer names): the content lead decides whether to fix, reframe or drop it
Rules it follows
- No protected health information enters prompts, model context or logs. Source material is de-identified before the model call, quarantined items never reach the model, and the model provider and any log storage operate under a signed BAA as a second layer of protection, not the first
- No clinical, outcome or efficacy claim publishes without a linked source in the claims ledger and a named clinical reviewer's approval recorded against that specific claim
- The agent never gives or implies individual medical advice, and every patient-facing topic is written for the professional buyer, not the patient
- Customer stories, quotes and logos are used only when a signed permission is on file in the evidence library, and no customer data is inferred from Salesforce
- Evidence items carry an expiry date, and the agent will not cite a study or guideline past its review date or one superseded by a newer version
How you know it works
- Posts published per business day that pass QA and sign-off at the agreed quality bar
- Non-branded keyword rankings and organic sessions to blog content, tracked in Moz and Search Console
- Share of tracked buyer questions where AI answers cite the company's content, from Rank Prompt and OmniSEO
- Clinical review turnaround time and first-pass approval rate for Tier 2 and Tier 3 drafts
- Opportunities in Salesforce with blog content touches, and content-influenced pipeline by audience segment
From pilot to live
- Pilot (weeks 1 to 4): Tier 1 only, three posts a weekBuild the redaction step, evidence library, brand voice guide and claims ledger. Publish only operational topics so the team can tune voice and QA without clinical review load. Confirm BAAs, logging settings and quarantine flow with the privacy officer before any real source material flows.
- Expand (weeks 5 to 10): add Tier 2 and Tier 3, move to dailyTurn on clinical and compliance routing, pre-approve a bank of reusable claim language with the clinical reviewer so routine claims clear faster, and connect Salesforce campaign tracking. Hold daily cadence only once review turnaround keeps up.
- Operate (ongoing): managed run with monthly tuningThe operating team monitors QA failure reasons, reviewer edits, rankings and AI citations, updates prompts and the evidence library, runs the refresh queue, and reports pipeline influence monthly. Quarterly, the privacy officer audits redaction logs and quarantined items.

Straight answers.
Can we feed it support tickets and call recordings that mention patients?
Not directly. Raw tickets and transcripts go through a de-identification step first, and anything flagged as possible PHI is quarantined for the privacy officer instead of reaching the model. Even with a BAA in place with the model provider, the design keeps PHI out of prompts and logs entirely, so the agent works from aggregated themes like recurring buyer questions, not individual cases.
Our one clinician already has a backlog. Won't daily publishing just bury them?
That is why drafts are tiered. Most daily posts are operational or regulatory topics that never reach the clinician. Tier 3 posts arrive with a claims ledger so the reviewer checks specific statements against sources rather than editing prose, and claims they approve once are saved as reusable language. Cadence only increases when review turnaround holds.
How do we know it is working beyond more posts going live?
Track three things: whether pages rank for non-branded buyer queries in Moz and Search Console, whether AI answers start citing your content for the questions health system buyers ask (measured in Rank Prompt and OmniSEO), and whether opportunities in Salesforce show blog touches. If volume rises but rankings and citations do not, the topic selection or structure gets tuned before cadence goes up further.
