HIPAA-aware AI support agent for small healthcare software vendors on HubSpot absorbing the January renewal surge from clinic admins

The Support Specialist is a HubSpot Breeze Customer Agent, fronted by a PHI-screening layer, that answers the seat, login, invoice, PO and new-year setup questions clinic office managers send after the annual renewal push, and completes the routine ones through actions into the licensing and billing systems. It keeps protected health information out of prompts and logs, sends anything clinical, coding-related or contractual to a person, and lets a small support team get through January without doubling headcount.

THE JOB

What The Support Specialist does

A 30-person company sells practice software to independent specialty clinics and outpatient groups, and almost every contract renews in December. Every January the support queue roughly doubles: office managers need seats added for new staff, invoices reissued with this year's PO number, a fresh W-9 or copy of the signed BAA, login resets after the holiday break, and help turning on the features they just paid for. A four-person support team handles this in HubSpot Help Desk while also fielding the hard tickets, and clinic staff often paste patient names, dates of birth or screenshots of charts into their messages. The Support Specialist works for that team. It owns first response on chat, the help desk inbox and the portal form; it screens every inbound message for PHI before the AI sees it; it answers from an approved knowledge base; it performs a short list of safe account actions (seat adds within contract, invite resends, invoice and document copies); and it hands everything else to the right person with a clean, PHI-free summary. It does not give clinical, coding or reimbursement guidance, and it does not change contract terms.

PLATFORM

Built on HubSpot

The team already runs tickets, contacts and renewal deals in HubSpot, so the Breeze Customer Agent can read contract and seat context natively, live in the existing help desk and chat, and be extended with custom actions into licensing and billing without a separate help desk. Where HubSpot falls short for a healthcare vendor is PHI control, so n8n self-hosted in the company's own BAA-covered environment does the screening before any message reaches the agent, and handles the API calls to systems HubSpot cannot reach directly. If a future need requires reasoning over content that may contain PHI, that piece should be a custom Claude build running through a cloud provider that offers a BAA, kept separate from the HubSpot agent.

TRIGGERS

What starts it

  • A new ticket arrives in the HubSpot help desk inbox from email or the portal support form
  • A clinic admin opens chat inside the customer portal or the logged-in help center
  • A renewal deal moves to Closed Won in HubSpot, prompting a proactive welcome-back message with seat and setup links for the new contract year
  • A ticket sits unanswered past the first-response SLA during the January surge window
  • A customer replies to an agent-handled ticket after it was marked resolved
DATA

What it reads

  • HubSpot knowledge base articles, tagged as approved by support and, for anything near clinical workflow, by the clinical reviewer
  • HubSpot company, contact and deal records (contract tier, seat entitlement, renewal date, named admins)
  • Licensing and user admin API of the product (read seat counts, add seats within entitlement, resend invites, trigger password reset emails)
  • Billing system for invoices, payment status, PO fields and tax documents
  • Document library of current BAA template, W-9, security questionnaire answers and SOC report request process
  • Help desk ticket history from the last two Januaries, de-identified, used for testing and gap analysis
THE WORKFLOW

How it works, step by step

  1. Screen for PHI before the agent reads anythingEmail and form tickets pass through an n8n workflow (self-hosted in the company's own BAA-covered cloud environment) that detects names with dates of birth, MRNs, insurance IDs and attachments that look like chart screenshots. Flagged tickets are quarantined from AI, the customer gets an automatic note asking them not to send patient details through support, and a human picks up the ticket. Chat shows the same warning before the first message and a Breeze guideline refuses to process patient identifiers.
  2. Identify the clinic and its contract yearThe agent matches the sender to a HubSpot contact and company, pulls tier, seat entitlement and renewal date, and confirms the person is a named admin before any account action. Unknown senders get answers to general questions only.
  3. Answer the January repeat questions from approved contentUsing only tagged knowledge base articles, the agent handles new-year setup, feature activation steps, login help, user role questions, where to find training, and how to request a copy of the BAA or W-9. If no approved article covers it, the agent says so and routes rather than improvising.
  4. Complete safe actions through connected systemsThrough Breeze custom actions calling the licensing and billing APIs, the agent resends user invites, triggers password reset emails, adds seats only up to the contracted count, reissues invoices with an updated PO number and sends the current W-9. Each action is logged to the ticket with the before and after values, never with patient data.
  5. Route everything else with a clean case summaryRequests for seats beyond entitlement go to the account manager as an expansion task, billing disputes to finance, bugs to the support lead with steps to reproduce, and any clinical, coding or reimbursement question to the clinical reviewer. The summary is written from the screened text only.
  6. Close the loop and measureResolved tickets trigger a one-question CSAT survey through Zonka Feedback. Ticket outcome, handle time, Breeze credit use and handoff reason feed a Databox dashboard showing deflection, cost per resolution, CSAT and credits, with a January view compared against the prior year.
HUMANS IN THE LOOP

When a person takes over

  • Any ticket the PHI screen flags goes to a human agent, who handles it inside the BAA-covered system and coaches the clinic on safe submission; the AI never sees it.
  • Questions about clinical workflow, medical coding, documentation requirements or what the software means for patient care go to the clinical reviewer, because clinical claims need qualified review before anything is said.
  • Seat requests above contract, pricing questions, cancellation intent or requests to change BAA terms go to the account manager or the contracts owner with the renewal deal attached.
  • Reports that data looks wrong, missing or exposed in the product go immediately to the support lead and the privacy officer as a possible incident, not into the normal queue.
GUARDRAILS

Rules it follows

  • No PHI in prompts or logs unless every system touching it is under a signed BAA: confirm whether your HubSpot tier and Breeze features are covered, and if any piece is not, keep the n8n screening layer mandatory and turn off conversation logging of raw text for flagged tickets.
  • The agent makes no clinical, coding, reimbursement or regulatory compliance claims; it may link only to articles the clinical reviewer has approved, and approvals carry a review date.
  • Account actions are limited to an allowlist (invite, reset, seats within entitlement, document and invoice copies), require a verified named admin, and never delete users or change permissions.
  • The agent never confirms whether a specific patient, provider or record exists in the system, even to a verified admin.
  • Every answer cites the article or record it came from, and the agent hands off rather than guesses when confidence is low or two sources conflict.
KPIS

How you know it works

  • Share of January tickets resolved without a human (deflection), tracked weekly against the prior January
  • Cost per resolution, combining Breeze credits and human handle time
  • CSAT on agent-resolved tickets versus human-resolved tickets
  • First response time during the January surge window
  • PHI screen catch count and false negatives found in weekly audit (target is zero misses reaching the AI)
ROLLOUT

From pilot to live

  1. Pilot in the fall, on last January's ticketsBuild the PHI screen, approved article set and four custom actions. Replay a de-identified sample of last January's tickets through the agent in a sandbox, have the support lead grade every answer, and have the clinical reviewer sign off on anything near clinical content. Fix gaps before December renewals close.
  2. Portal chat and help form in early DecemberTurn the agent on for logged-in portal chat and the help form, where identity is known and volume is still normal. Humans review every agent reply for the first two weeks, then sample daily. Confirm credit usage matches the forecast before the surge.
  3. Email inbox for the January surge, then year-roundAdd the shared help desk inbox the week after renewals close, with the proactive welcome-back message on Closed Won renewals. Run a weekly PHI audit and article refresh through January, then keep the agent on all channels year-round with a quarterly content and guardrail review.

How we build The Support Specialist (customer agent) ↗

Questions about this agent

Good questions.
Straight answers.

Clinic staff paste patient details into tickets all the time. How do we stop that from reaching the AI?

Every email and form ticket is screened by an n8n workflow running in your BAA-covered environment before the agent sees it. Anything that looks like PHI is held back from AI and handled by a person, and the customer gets a short note on how to submit safely. Chat shows a warning up front and the agent refuses patient identifiers. You also confirm in writing which vendors, including HubSpot, are covered by your BAAs, and you design around any gaps rather than assume coverage.

What happens when a clinic asks how to document or code something in our software?

The agent can explain where a field is and how a screen works using approved how-to articles, but it does not say what a clinic should document, which code to use, or what will be reimbursed. Those questions go to your clinical reviewer with the ticket summary, and any answer that proves reusable becomes a reviewed article with an expiry date.

Will this actually hold up when volume doubles in January, and who keeps it working?

It is built and tested in the fall on last January's de-identified tickets, live on chat in December, and on email the week the surge starts, so the busiest weeks are not the first weeks. Under a build and maintain arrangement, someone owns the weekly PHI audit, article updates, action fixes when the billing or licensing APIs change, and the credit and cost reporting, so your support lead reviews results instead of maintaining the plumbing.
